A source conflict is a stop signal

If a chat message, site photo, old drawing or informal instruction conflicts with a current approved source, do not average the answers or choose the convenient one. Preserve both, identify their authority and revision, assign resolution, and hold affected work where safety, compliance, quality or irreversible cover-up is at risk.
